Offer valid only on the following ZYRTEC® products: ZYRTEC® 10mg Tablets 3, 5, 14, or 30 count sizes; ZYRTEC® 10mg Liquid Gels 12 or 25 count sizes; any Children’s ZYRTEC®; any ZYRTEC-D®. A refund check will be issued for the original purchase price. The cost of first class postage stamp will be reimbursed in refund check. Offer open to U.S. residents only. Limit one (1) refund per household (except in Rhode Island). All refund requests must be postmarked by 1/31/2011. Offer void where prohibited. Not valid with any other offer. Not transferable or substitutable. Duplicate, incomplete or invalid requests and/or requests from clubs/organizations are void and will not be processed or returned. Requests from P.O. Boxes will not be honored or returned (except for residents of North Dakota). McNeil Consumer Healthcare, Division of McNeil-PPC, Inc. (“McNeil”) reserves the right to cancel, rescind or extend this offer at any time. Submitted materials, receipts and UPCs must be originals and are subject to verification. No photocopies or duplicates of UPCs or receipts allowed and no other brands or products will be accepted. Sale, trade, or purchase of UPCs or receipts is prohibited. All requests become property of McNeil. Return of any check as undeliverable will result in forfeiture. Check void if not cashed within three (3) months of date of issue. McNeil, its parents, affiliates, subsidiaries, promotional agencies and retailers (collectively, “Releasees”) are not responsible for lost, late, incomplete, inaccurate, stolen, misdirected, undelivered, mutilated, illegible, or postage due requests, receipts, UPCs or mail, or other errors or problems of any kind whether human, mechanical, typographical, printing or otherwise relating to or in connection with the offer, including, without limitation, errors or problems which may occur in connection with the administration of the offer, the processing of requests, or in any offer-related materials. Releasees are also not responsible for any incorrect or inaccurate information. Persons who tamper with or abuse any aspect of the offer or who are in violation of these Terms, as solely determined by Releasees, will be ineligible to receive the offer. Please allow 8-10 weeks for delivery of refund check. Please print and return this form along with the original UPC from your ZYRTEC® package and the original store receipt dated between 8/1/10 and 12/31/10 with purchase price circled.
